The client portal is for FreshWave clients with an active policy. Tell us who you are and a FreshWave team member will verify your identity, usually by calling the phone number on your policy record, before sending an activation invitation to the email address on that record. Sending this form doesn't give access to any policy information.

Privacy notice at collection

What we collect:: Your name, email address, telephone number, and state, and, only if you choose, the last four letters or numbers of a policy number. Please don’t enter a full policy number, Social Security number, date of birth, or password.
Why:: To find your FreshWave client record and verify your identity before we activate client portal access. Portal access is only for clients whose identity we have verified and who have an active policy placed through FreshWave. A FreshWave team member verifies you, for example by calling the phone number already on your client record, or by checking government photo ID in person or on a live video call, and may contact you if more information is needed. Sending this form does not give you access to any information and does not guarantee that an account will be activated. We send an automatic email confirming that we received your request; it does not say whether you are a FreshWave client. If we activate your account, the invitation goes to the email address already on your client record. We keep this request, our decision, and a record of how and when your identity was verified and by whom; for portal activation we never keep copies of ID documents or ID numbers. Requests we cannot match or verify are declined, without an automatic email, and kept as described in our Privacy Notice.
Sharing:: Not sold and not shared with insurance companies or partners. It is stored and processed for us by our hosting, database, and email-delivery providers, only to run our systems.
